Purpose
The information is used to respond to enquiries, understand interest in the DentiRelay pilot, arrange demonstrations or testing, and communicate about participation in the pilot.
Legal basis
DentiRelay processes the information on the basis of its legitimate interest in responding to enquiries that dental practices choose to send about the pilot, and in arranging the demonstrations or testing they ask about (Article 6(1)(f) of the General Data Protection Regulation).
This basis is used only to respond to voluntary pilot enquiries. DentiRelay does not use enquiry information for marketing, newsletters or other promotional messages, and does not add it to mailing lists.
Only business contact details are used, and only for the purpose you contacted DentiRelay about. DentiRelay has assessed that this use is necessary to reply to you and does not override your interests or rights, because you chose to make the enquiry and would expect a reply. A summary of this assessment is available on request from info@alexjsolanke.me.
Providing your details is voluntary, but the required fields are needed to respond to your enquiry.
Your right to object
You can object at any time. You have the right to object to DentiRelay processing your information on the basis of its legitimate interest (Article 21 of the General Data Protection Regulation). To object, email info@alexjsolanke.me. DentiRelay will then stop using your information to respond about the pilot and will delete it as described under “Retention”, unless it must be kept to comply with a legal obligation or to establish, exercise or defend legal claims.
Patient information
Business contact information only. The form is for business contact information only. Visitors must not provide patient names, health information, call recordings, treatment details, or other patient information.
Service provider
Form submissions are processed through Formspree, which receives and stores them on behalf of DentiRelay and forwards them by email.
Formspree states that its services are hosted with Amazon Web Services in the United States, so information you submit is transferred to and processed in the United States. Formspree states that it relies on Standard Contractual Clauses as a data processor. To ask about the safeguards that apply to this transfer, contact info@alexjsolanke.me.
Retention
Pilot enquiry information may be retained for up to 12 months after the last contact, unless it must be retained longer for an established business relationship or legal obligation. It may be deleted sooner on request where applicable.
This period applies to every copy DentiRelay holds:
- the submission stored in DentiRelay’s Formspree account; and
- the enquiry emails in the DentiRelay mailbox (info@alexjsolanke.me), which is hosted by Titan, including any replies.
DentiRelay reviews pilot enquiries at least once a month. When an enquiry reaches the end of its retention period, or when a deletion request or objection is accepted, DentiRelay deletes the submission from its Formspree account and deletes the related emails from the mailbox, including sent replies and the deleted-items folder. Requests are answered within one month of receipt.
Choices and requests
A person may request access, correction, or deletion of their submitted information, or object to its use (see “Your right to object” above), by emailing info@alexjsolanke.me.
You may also ask for the processing of your information to be restricted.
Complaints
If you believe your information has been handled in a way that does not comply with data protection law, you have the right to lodge a complaint with the Commission nationale de l’informatique et des libertés (CNIL), the French data protection authority:
- online: www.cnil.fr/fr/plaintes (opens in a new tab)
- by post: CNIL, Service des plaintes, 3 Place de Fontenoy, 75007 Paris, France
You may also complain to the data protection authority in the European Union country where you live or work, or where you believe the problem occurred. You are welcome to contact info@alexjsolanke.me first so the concern can be looked into.
